Have you used the Guide to Community Preventive Services (Community Guide) to address public health challenges in your community? If so, the Public Health Foundation (PHF) wants to hear from you!

PHF invites you to share your experience by entering the “I’m Your Community Guide” Contest . Your stories will help to identify model practices for implementing the evidence-based strategies outlined in the Community Guide.

Stories are encouraged from all state, tribal, local, and territorial health departments; community health coalitions; and other organizations that have used or are currently using the Community Guide’s findings and recommendations to improve health in their communities. The top 3 entrants will each be awarded a scholarship (up to $1,000) toward attendance at a national public health conference in 2011. The top 15 entrants will win an “I’m Your Community Guide” prize pack, and all entrants will receive 15% off their next purchase of public health training materials from the PHF Online Store.

Stories may be submitted via e-mail to olivia@phf.org through May 6, 2011. Submissions may be made as a text document of no more than two pages or as a completed contest entry form. Stories will be made public to create a community of learning and sharing surrounding the Community Guide. Complete submission information is detailed in the “I’m Your Community Guide” Contest Submission Guidelines .
